- Dilation
- Effacement (thinning of the cervix)
- Cervical consistency
- Cervical position
- Fetal station (baby’s position in the pelvis)
Each element receives a point value, and the total score helps your care team understand whether your body is already moving toward spontaneous labor or whether it may need a little more time.

Breaking Down the Five Components
- Cervical Dilation
This reflects how open the cervix is, from 0 to 10 centimeters.
Early dilation can happen slowly over days or weeks, especially for first-time mammas, and that gradual unfolding is completely normal.
- Effacement
Effacement describes how thin and stretchy the cervix has become.
First-time mammas often efface before they dilate; mammas who’ve birthed before may dilate earlier. Both patterns are healthy and normal.
- Cervical Consistency
Your cervix naturally changes from firm to soft as it prepares for birth, becoming more flexible and responsive to contractions.
- Cervical Position
As labor approaches, the cervix shifts from pointing toward the back of the pelvis to moving forward, making it easier for baby to engage.
- Fetal Station
This measures how low your baby’s head sits in your pelvis.
